The discs that I received were tracked pretty much this way, but no fade tracks.
Right at the end of the last track of disc #1, and right at the beginning of the
first track of disc #2, there were a small number of silent samples. I removed
those silent samples, ran shntool fix *.wav to correct the .wav file lengths for
these and all subsequent tracks, then prepared the fade tracks. d1t05b and d2t00
are redundant if burning to longer media (ie DVD). You don't have to use them if
burning a conventional 2-disc layout, but they make the changeover smoother when
you flip CDs.

All tracks were verified with shntool that they are cut to correct CD sector
boundaries.
