Notes:
- d2t03: deck was stopped to change tapes just after the song ended.
I edited the splice point back to just after John says "Thank you"
  for a near-seamless transition.
- d3t05 is cut on the DAT at 6:50 and does not resume; I patched in from an
analog master which has the same cut but DOES have the rest of the song.
I inserted two seconds of silence before cutting the analog in to keep it
from sounding like a skip. The analog master was only used for this one
track; the encore is picked back up from the DAT.
