Notes:

- First 70 seconds of #41 are cut (not on original DAT)
- First 30 seconds or so have really high levels
- Slight level adjustments during first 02:30 of #41

Fixes:

- This show was not perfect.  There were problems with the right channel throughout
  due to a defective mic.  To fix this problem, trouble spots in the right channel
  were replaced with a clone of the left channel.  Specifically:

- First 30 seconds of #41 had problems with right channel; left channel cloned
- 30 seconds of Rhyme & Reason had problems with right channel; left channel cloned
- Last minute of Recently and all of Ants Marching into the encore break had
  problems with the right channel; left channel cloned
